Authorization for Background and Consumer Reports

I hereby authorize Netsirv, LLC to obtain background check reports and consumer reports for the purpose of evaluating my application for employment and, if applicable, my continued employment. These reports may be obtained through a third-party service provider and may include, but are not limited to, information regarding my driving record and an assessment of my programming skills and professional experience, as permitted by law.

By signing below, I give my ongoing consent for Netsirv, LLC to request and review such reports both at the time of application and periodically thereafter, as necessary, for legitimate business purposes and in accordance with all applicable laws and regulations.
